Letter to 5th Grade Parents from School Nurse
The following are requirements for entering grade 6 in the Fall of 2022 as per the NJ State Department of Health and Department of Education.

1:IMMUNIZATIONS

·        DTAP Vaccine

·        Meningococcal Vaccine

If you have questions about these vaccines, please contact your health care provider.  If you do not have a healthcare provider, you can contact the Mt. Olive Board of Health for free or reduced cost immunizations.  The MO Health Dept. can be reached at (973) 691-0900 x7353  to schedule an appointment.

2: PHYSICAL

·        Including documentation of above immunizations

·        Scoliosis

·        Blood Pressure /Height / Weight

Also, if your child will be potentially participating in extracurricular  activities during the 6th grade year the school he or she will REQUIRE “sports physical” documentation. .  Please have your doctor complete the athletics physical form which can be found by clicking this link: http://www.mtoliveboe.org/moms/departments/healthOffice.php?Sports-Physicals-5  or http://www.mtoliveboe.org/departments/healthNursing.php?Medical-Forms-2

Please be aware that if requirements are not met your child will not receive their schedule and will be excluded from beginning school in September 2018 as per NJ State Administrative Code 16.

Please submit all required documentation to your child’s current school nurse or to the MOMS nurse prior to August 20, 2022 in order to make for a smooth transition into the Mount Olive Middle School.
